If your student is eligible for testing accommodations through a 504 Plan, IEP, or English Language Learner status (LY)) please complete these forms to receive their approved accommodations for national standardized testing. Accommodations must be requested and approved in advance through College Board and ACT. If they are not approved, they cannot be given. Please complete and return these forms as soon as possible. Forms should be returned to the front office or the Testing Office, Room 3-127 or the Front Office. Forms received after the deadlines listed below will not be guaranteed approved accommodations but will be requested.
*Please note: If your child does not wish to receive accommodations, please do not complete this form. Any accommodations requested and approved must be implemented. This may cause your child to be testing the entire length of the school day, depending on the extra time given.

- All 504 Plan and IEP Students are eligible to request accommodations. Approved accommodations may vary slightly from those listed on the 504 Plan or IEP.
- ELL language supports will only be provided for the SAT School Day (12th Grade).
- ELL students taking the PSAT/NMSQT (9th – 11th Grades) are not eligible to receive language supports.
